My required form fields aren't requiring answers prior to submission

This is an indication that the Hidden Field containing your required field names doesn't match the actual field names.  To fix this, right-click on the Hidden Field and select Field Properties from the list, then verify that each field name is spelled exactly the way it was spelled when you created that form field.  Also, be sure that the field names are separated only by commas, with no spaces in between.
